19th Annual Chronic Illness and Disability Conference *Region One ESC Broadcast Site*

*Two day conference broadcast.* Skill preparation and planning for health care transition for children and youth with special education healthcare needs is inadequate. Less than 50% of families nationwide indicate that their children with special healthcare needs have received the services necessary to make appropriate transitions to adult healthcare, work, and independence. At the conclusion of the conference, participants will leave with the ability to describe strategies to identify barriers and successful strategies when placing youth and young adults with special healthcare needs, behavioral health, and intellectual disabilities into employment, housing, and available medical transition services.

Parents, guardians, special education high school teachers, nurses, physical therapists, occupational therapists, social workers, case managers, counselors, and young adults who have a chronic illness or disability.
