Transition services provide a pathway to help students achieve their postsecondary goals. In this three-part session, participants will explore the differences between annual and postsecondary transition goals, with a focus on the three required postsecondary domains: education/training, employment, and independent living. Participants will also learn about age-appropriate transition assessments, coordinated sets of activities, and how to write measurable postsecondary goals. Through practical resources, activities, and required coaching, participants will strengthen their ability to select appropriate transition assessments, develop meaningful coordinated sets of activities, and write measurable postsecondary goals. This coaching is designed to improve practice, build capacity and confidence, and ultimately enhance postsecondary outcomes for students, and participants agree to engage in coaching session(s).
